Development of Diabetes Care Guidelines of Ban Bua Sub-District Health Promotion Hospital by using the Chronic Care Model

Abstract

The purpose of this action research was to analyze Situation of Diabetic Care and Development of Diabetes Care Guidelines of Ban Bua Sub-District Health Promotion Hospital by using a chronic patient care model. The samples were diabetic patients, village health volunteers, and public health officers at the Health Promotion Hospital. Data were collected by the tool contains 1) interview form for diabetes patients, health volunteers and and public health officers 2) questionnaires for group .discussions  for diabetes patients, health volunteers and 3) meeting record forms. Validate the quality of the tool by finding content validity and Cronbach's reliability was .88. Data were analyzed using descriptive statistics and paired-t test.

     The results showed that Knowledge of diabetes of most diabetic patients was at high level , representing 94.83 %. The patient's self-care behavior was at a good level. Guidelines for diabetes care include: 1) annual blood testing, retinal examination, Check for foot numbness 2) Adjust the treatment by the medical team annually 3) Empowerment by using 4S to reduce blood sugar levels in patients with poor blood sugar control 4) Repeat follow-up appointments in patients whose blood test results The patient's blood sugar level was monitored every 3 months. Clinical results after using the guideline for diabetes care showed that the mean cumulative blood glucose (HbA1C) before and after the guideline was different. statistically significant at the .009 level
