Developing an integrated diabetes care system towards excellence Nonghan Hospital

Abstract

     This research is action research. The objective is to develop a diabetes care system for nursing homes in Nong Han Hospital. Study the effectiveness and satisfaction with the diabetes care system. Nong Han Hospital that has developed A sample of 90 people were diabetic patients with blood sugar levels greater than 130 mg/dL. And there are no complications from being diabetic. Divided into an experimental group and a comparison group, 45 people per group. The experimental group used the developed patient care system. The comparison group used traditional care. Tools used include Diabetes care system, questionnaires, and health status records. Data were analyzed using descriptive statistics, Chi-square test, Independent t-test, and paired sample t-test.

     The results of the study found that the developed patient care system consists of a nursing process divided into 3 phases: Pre-Hospital, In Hospital, and Post-Hospital, organized through a rapid and safe patient referral system. and complete records of care information supporting continued medical treatment The results of using the developed system found that after the experiment, diabetic patients had better self-care behavior both overall and in each aspect. Blood sugar levels decrease Statistically significant difference (p-value <0.001) for body mass index. and waist line There was no statistical difference. When comparing within groups, it was found that the experimental group had better self-care behaviors both overall and in each aspect. Blood sugar levels and body mass index decreased significantly from before the experiment (p-value <0.001). As for waist circumference, Not statistically different Diabetes patients who received care with the developed system, 86.7 percent, were very satisfied.
