Development of a care model for the elderly with dependency by caregivers in the community Community Health Center, Phon Phisai Hospital, Phon Phisai District, Nong Khai Province.

Abstract

     The objective of this action research was to develop a model and study the results of a model for caring for dependent elderly among community caregivers at Phon Phisai Hospital Community Health Center, Phon Phisai district, Nongkhai province. Divide into 3 phase: 1^st phase studies the problem,2^ndphase develops the model and 3^rdphase evaluates the results of the developed model. The sample group consisted of 19 elderly caregivers (Caregivers:CG) of the Phon Phisai Hospital Community Health Center and relatives of 30 dependent patients, Data were collected by focus group discussion. Inquiry of knowledge and evaluate satisfaction. Content analysis descriptive statistics were used: frequencies, percentages, means, and standard deviations and inferential statistics, paired t-test
     The results of the study found that the Phon Phisai Hospital Community Health Center does not have a clear care model. CG lack of knowledge and skills in caring for patients and lack of knowledge in using screening forms and recording various reports. These causes a lack of confidence in caring for patients in the community. The developed format consist of (1) Care plan (2) health record (3) activities of visiting for 2 hours each time, 4 times a month, care until the elderly pass away, (4) coordination of involved agencies, and (5) schedule a CG meeting once a month to reports and exchange knowledge about problems and obstacles that arise. After applying the developed model, it was found that CG's knowledge increased significantly. (p-value<0.001), relatives' knowledge increased significantly. (p-value<0.001) and dependent elderly people had a 46.7 percent increase in their ADL scores. No complications or infections were found. In addition, CG and relatives were satisfied with the developed model at a good level.
