บทคัดย่อ

     การศึกษานี้เป็นการวิจัยเชิงสำรวจ บทความปริทัศน์เชิงบรรยายร่วมกับกระบวนการสืบค้นอย่างเป็นระบบ มีวัตถุประสงค์เพื่อสังเคราะห์หลักฐานเกี่ยวกับผลของการเสริมครีเอทีนต่อปริมาณน้ำในร่างกายโดยรวม (total body water, TBW) น้ำภายในเซลล์ (intracellular water, ICW) และน้ำภายนอกเซลล์ (extracellular water, ECW) รวมถึงวิเคราะห์ตัวแปรร่วมที่ส่งผล ได้แก่ การเสริมครีเอทีนแบบการให้ขนาดสูงระยะสั้น (loading phase) กับการเสริมในระยะคงระดับ (maintenance phase) ขนาดและระยะเวลาการเสริม เพศกำเนิด สถานะการฝึกซ้อมการออกกำลังกายอย่างเป็นระบบ และวิธีการวัดมวลน้ำ โดยสืบค้นจากฐานข้อมูล PubMed และ Google Scholar เฉพาะการศึกษาในมนุษย์ ได้ผลการสืบค้นทั้งหมด 2,970 การศึกษา หลังผ่านการคัดกรอง มีบทความนำมาทบทวนรวมทั้งสิ้น 12 การศึกษา ใช้การวิเคราะห์เชิงพรรณนาโดยการเปรียบเทียบขนาดและลักษณะกลุ่มตัวอย่าง ขนาดและระยะเวลาการเสริมครีเอทีน วิธีการวัดปริมาณน้ำในร่างกาย และตัวแปรร่วม โดยแบ่งกลุ่มตามเพศ สถานะการฝึกซ้อมการออกกำลังกายอย่างเป็นระบบ ขนาดและระยะเวลาการเสริมครีเอทีน

     ผลการศึกษาพบว่า การเสริมครีเอทีนส่งผลให้ปริมาณน้ำในร่างกายโดยรวม (TBW) เพิ่มขึ้นประมาณ 1–2 ลิตร ซึ่งเกิดขึ้นในส่วนของน้ำภายในเซลล์ (ICW) เป็นหลัก ประมาณ 0.6–1.0 ลิตร และเกิดขึ้นในส่วนของน้ำภายนอกเซลล์ (ECW) ประมาณ 0.46 ลิตร โดยพบว่า ขนาดของการเสริมครีเอทีนในระยะคงระดับในช่วง 3–5 กรัมต่อวัน สามารถส่งผลต่อการเพิ่มปริมาณน้ำในร่างกายได้ สอดคล้องกับคุณสมบัติออสโมติกของครีเอทีนที่ดึงน้ำเข้าสู่เซลล์กล้ามเนื้อ ส่วนตัวแปรร่วมที่ศึกษาพบว่า เพศชายตอบสนองต่อการเพิ่มขึ้นของมวลน้ำได้มากกว่าเพศหญิง ขณะที่เพศหญิงที่อยู่ในช่วงระยะ luteal phase ของรอบประจำเดือน มีปริมาณน้ำในร่างกายเพิ่มขึ้นมากที่สุดหลังการเสริม ในทางกลับกัน ไม่พบความแตกต่างของอย่างชัดเจนของการเพิ่มปริมาณน้ำในร่างกายเมื่อพิจารณาตามขนาดของการเสริม ระยะเวลาที่ใช้ในการเสริม รวมถึงสถานะการฝึกซ้อมการออกกำลังกาย
