บทคัดย่อ

     การวิจัยเชิงปฏิบัติการครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสภาพปัญหา พัฒนารูปแบบและประเมินผลรูปแบบการดำเนินงานเฝ้าระวัง ป้องกัน และควบคุมวัณโรคเครือข่ายสุขภาพจังหวัดหนองบัวลำภู ผู้ให้ข้อมูลสำคัญเป็นผู้ให้บริการสุขภาพ 120 คน ผู้เชี่ยวชาญตรวจคุณภาพเครื่องมือวิจัย 7 คนและกลุ่มเสี่ยง 24,739 คน ดำเนินการวิจัยตั้งแต่เดือนพฤศจิกายน 2568–สิงหาคม 2569 เก็บรวบรวมข้อมูลโดยใช้แบบบันทึก แบบสอบถามและแนวคำถามสำหรับการสนทนากลุ่มที่มีความตรงเชิงเนื้อหา (IOC) ตั้งแต่ 0.80-1.00 วิเคราะห์ข้อมูลโดยใช้ความถี่ และร้อยละ

     ผลการวิจัย (1) การดำเนินงานที่ผ่านมาพบปัญหาสำคัญใน 3 มิติ ได้แก่ 1) ด้านการคัดกรอง: ยังตกหล่นในกลุ่มประชากรแฝง แรงงานเคลื่อนย้าย และกลุ่มเป้าหมายเข้าถึงยาก 2) ด้านการวินิจฉัย: รพ.สต. ยังขาดแคลนเครื่องมือวินิจฉัยรวดเร็ว ทำให้ค้นหาผู้ป่วยได้ล่าช้า และ 3) ด้านการดูแลรักษา: ผู้ป่วยบางส่วนขาดความต่อเนื่องในการรับประทานยา จนเพิ่มความเสี่ยงต่อภาวะรักษาล้มเหลวและการเกิดเชื้อดื้อยา (2) รูปแบบฯ ที่ได้ประกอบด้วยกิจกรรมสำคัญ 6 ขั้น คือ (1)  การวางยุทธศาสตร์ และจัดสรรทรัพยากร (2) การคัดกรองเชิงรุกและส่งต่อชุมชน (3) การตรวจวินิจฉัยแม่นยำด้วยเทคโนโลยี  (4) การกำกับการรักษาและดูแลในรายที่ซับซ้อน (5) การบริหารจัดการข้อมูล Real-time และ(6) การทบทวนคุณภาพและนิเทศติดตาม และ (3) ผลการประเมินรูปแบบฯ พบว่า อัตราความครอบคลุมของการคัดกรองวัณโรคเชิงรุกในกลุ่มประชากรเสี่ยงสูงด้วยการถ่ายภาพรังสีทรวงอก (CXR) คิดเป็นร้อยละ 95.78 (34,493 จากเป้าหมาย 36,014 คน) อัตราความครอบคลุมการขึ้นทะเบียนผู้ป่วยวัณโรครายใหม่และกลับเป็นซ้ำ คิดเป็นร้อยละ 66.61 (465 จากเป้าหมายคาดประมาณ 698 คน) และ3) อัตราความสำเร็จการรักษาผู้ป่วยวัณโรคปอดรายใหม่ ในกลุ่มประชากรผู้ป่วยวัณโรคปอดรายใหม่  คิดเป็นร้อยละ 96.25 (77/80 คน)
