Nursing Care of Patient Stroke in the acute phase : 2 Case Study

This case study aims to study nursing care for stroke patients who come to the service in the acute phase. By comparing two case studies, purposively selected from patients diagnosed with stroke in the acute phase, with symptoms occurring within 4.5 hours, using the concept of patient assessment according to Gordon's 11 health plans. To find problems and needs of patients Follow the 5-step nursing process. 1) Assessment of problems and needs (Assessment) 2) Nursing diagnosis (Nursing Diagnosis) 3) Nursing care plan (Nursing care plan) 4) Nursing practice (Nursing Intervention) 5) Evaluation of nursing practice results ( Evaluation)
The results were found: This study was a comparative study of acute stroke patients admitted to the Accident and Emergency Unit at Sirindhorn Hospital. Khon Kaen Province Patients were screened using B-E-F-A-S-T and admitted to the Stroke fast track. They were patients referred from a community hospital and had comorbidities. with elderly patients who come by themselves without entering the Stroke fast track system. Receive quick and safe management of emergency complications. Both cases were treated by giving the blood clot-dissolving drug rt-PA. Nursing care was provided for complications before and after receiving the blood clot-dissolving drug. Receive continuous care from a multidisciplinary team. Evaluate nursing results, the patient's symptoms improved and were discharged home. They will be referred to a community hospital and make an appointment to continue monitoring their symptoms.
