Nursing care for STEMI patients and Streptokinase treatment in Kalasin Hospital: 2 Case Study

Abstract

     This study was aimed to compare nursing care for patients with STEMI and Streptokinase (SK) treatment .This two case study of SK treatment patients with STEMI by purposive selection. Which Entered for treatment in the ward of Kalasin Hospital between October 2022- September 2023.amounting to 2 cases. The data were collected from medical record interviewed patients and their relative. The functional health pattern of Gordons 11 Concepts of Health Model was used to assess health problems to planning a holistic nursing care, nursing diagnosis, nursing care planning program and to evaluate the outcome of nursing care of SK treatment patients with STEMI. The results of the study found that

Case study 1: A 65-year old Thai female patient with Stroke non fast track underlying hypertension, dyslipidemia diagnosis. The patient was refer from Kharm Muang Hospital for prepare Stroke treatment. The patient had significant symptoms near unconscious don’t spoke and following orders. The patient had Glasgow coma scale were E3V4M5,left motor power grade 3,right grade 3.Admit in Kalasin Hospital. After at Kalasin Hospital patient was chest pain, breathing heavily, restless. EKG show LBBB with ST elevation lead V1-V4. The patient was diagnosis of STEMI inferior wall with Congestive heart fail (STEMI with CHF). The patient prepare STEMI treatment by SK. During drug administration patient was breathing heavily, restless, unconscious. The patient prepare by On ET-Tube with respirator. And after SK the patient was unresponsive and EKG show STEMI. The patient after that refer to KonKean Hospital for prepare treatment and high potential hospital.

Case study 2: A 61- year old Thai male patient with pain in the middle of the chest radiating to the left arm and back and orthopnea 4 hours before coming to the hospital. The patient was diagnosis of STEMI inferior wall. The patient was refer from Kalasin-Tonburee Hospital for prepare SK treatment at Kalasin Hospital. But after SK treatment don’t was just refer to Konkean Hospital because no heave bed and after was.
