Development of Tuberculosis patients with 3C model Namyuen hospital, Ubon ratchathani

The objective of this study is to develop the care system for tuberculosis patients to increase the success rate to more than 85 percent. Nam Yuen Hospital takes care of the population of 70,000 people and care for TB patients continuously. Problems in the care of tuberculosis patients include lower treatment rates and high mortality rates, patients lacking appointment, lack of medication, co-ordinating problems between the area and the hospital and the community did not see the importance of diseases.Various problems result in unsuccessful treatment rates according to the criteria of 2008 equal 75.0 percent, in 2009 equal 50.0 percent and in 2010 equal 71.2 percent. Brainstorming to find the root cause analysis is the lack of medication. Develop a successful and sustainable work system by designing and linking the 3 wheels, including the Client, Care team and Community by studying with every patients in the year 2011-2018 there were 495 patients treated after using the 3C model. It was found that the success rate of TB patients. Increasing as follows : Year 2011 equals 96.7 percent, 2012-2018 equal to 100.0 percent. In addition, other quality indicators have good prospects, death rate decreases, reduced rate of loss appointment, conclusion the activity driven by the 3C model can increase the success rate of tuberculosis patients and can be extended to other chronic patients.
