Abstract

     This action research aimed to develop a model for educating and supporting self-management of diabetic patients. and to study the results of an educational and self-management support model for patients with difficult-to-treat diabetes. in the diabetes clinic Mae Lao Hospital The sample group was type 2 diabetic patients receiving insulin injections. But still unable to control blood sugar levels There were 34 patients with cumulative blood glucose ≥10 mg/dl. The research procedures were (1) situation analysis; Maintain (3) Assessment of outcomes Qualitative data were analyzed by content analysis. Quantitative data analysis by descriptive statistics and test statistics.

     Results (1) Situation Analysis found problems with inappropriate health care behaviors and factors that hinder As a result, the sample group had hyperglycemia. (3) diabetes patient self-management
education and support standards; and (3) diabetes patient outcome assessment. It was found that the
average score of self-management behavior increased. clinical information Mean blood glucose and cumulative blood glucose values were significantly reduced (p < 0.001). The ability to take care of diabetes by yourself resulting in better disease control and help prevent complications that will occur which should be further studied in the long run
