Development of care model for Intermediate care stroke patients in Kranuan Crown Prince Hospital and the health service network in Kranuan District, Khon Kaen Province

This action research objective was to develop a model of care for intermediate-care stroke patients in Kranuan Crown Prince Hospital and Health Service Network, Kranuan District, Khon Kaen Province, and to study the results of the model’s implementation for patients with a stroke in the intermediate care. The sample consists of 1) The group that developed and utilized the model for Intermediate stroke patients, including a physician, physiotherapist, Thai traditional medicine physician, nutritionist, pharmacist, and 65 registered nurses in the hospital, 13 professional nurses in the sub-district health promoting hospital and Primary Care Unit, 39 volunteers, in a total of 117 people. 2) New intermediate care stroke patients from 1st October 2021 - 28th February 2022, overall 34 cases. Analyzing the quantitative data by descriptive statistics using numbers, percentages, and qualitative data for content analysis and triangular examination (Triangulation data). The results found that the intermediate care of stroke patients model consisted of 1) planning for intermediate care for stroke patients 2) visit and monitor regulation 3) home chart 4) the Line group for advising between patients, cousins, and authorities 5) innovation in patient care, the program for monitoring abs visiting the stroke patients in the health care network at Kranuan district, Khon Kaen province. The results are that all 34 patients received the scheduled visits, which represents 100%, and all of them were systematically monitored monthly for the Barthel index score evaluation report continuously until the end of 6 months Barthel index score increased from the original number of 33 patients, and 1 patient with the same Barthel index score Mean (SD) Barthel represents 97.05%, the ADL index is 41.6(17.3) after discharge 6 months, the mean Barthel ADL index is 73.8 (20.9), and no complications occurred.
