This research and development aimed to study the results of developing a self-management program with the participation of sub-district health promotion hospitals to control blood sugar levels in patients with type 2diabetes mellitus. The sample consisted of 100 diabetic patients receiving medication in 3 sub-district health promoting hospitals in Renu Nakhon District. . The research tools consisted of 2 parts: Part 1 Participatory self-management program of sub-district health promoting hospitals Part 2 Tools used to collect data consisted of a food consumption behavior questionnaire. Physical activity, drug use, and abbreviated World Health Organization quality of life indicators. data analysis General data were analyzed by using numbers, percentages, health behaviors, and abbreviated World Health Organization quality of life indicators using mean values. And the cumulative blood sugar level (HbA1c) using mean, Pair t-test statistics before and after joining the program.
The process of developing a self-management program with the participation of sub-district health promoting hospitals for controlling blood glucose levels in type 2 diabetic patients consisted of 4 steps: planning (Plan), action (Act), follow-up and evaluation ( Observe) and reflect the results of practice (Reflect). The results of the study found that After participating in the program, it was found that the food consumption behavior of the subjects mean scores increased (M=47.09, SD=3.05), physical activity scores increased (M=9.75, SD=2.88), and drug use increased the mean score (M=21.96, SD=2.12), The quality of life for physical health and social relations were moderate quality of life. Mental and environmental aspects have a good quality of life. Mean cumulative blood sugar (HbA1C) was lower (M=8.972, SD=2.15) than before joining the program (M=9.604, SD=2.67), significantly different level of .05
