Abstract

     This study was study of stroke patients after discharge from after receiving continuous care at home for more than 6 months (Long Term Care). The study was conducted between October 1, 2020 - December 31, 2021. The objective was to study the nursing care of stroke patients in the community. Cases of stroke patients with community caregivers and those without community caregivers Comparison of nursing care according to the nursing process.

     The study found Case study 1 was discharged home on August 8, 2020. September 2020, there was a community caregiver. Go to take care at home 4 hours a day every morning. except Sunday By doing physical therapy according to the Individual Care Plan that joins the care plan by a multidisciplinary team. Ubonrat Khuean Subdistrict The patient still can't speak clearly on oxygen cannula and inserting a urinary catheter. January 4, 2021 began practicing removing oxygen and urinary catheter. And began to communicate enough to understand. In February 2021, the patient still spoke clearly, in a good mood and began to learn to walk using a cane. July 16 – 22, 2021, fell from learning to walk alone. went to bed for treatment in Ubolratana Hospital in August 2021, walking further outside the house Able to call the name of the child correctly. The second case study patient, 2021, presented with drowsiness, speechlessness, right hemisphere weakness. He was sent to Khon Kaen Hospital for diagnosis of basal ganglion hemorrhage, non-surgical treatment. physical therapy control sugar levels and control blood pressure within the criteria But this time the weakness is not better. Right limb weakness level 2. Patients stuck in bed have frequent complications. and has entered palliative treatment.
