This research and development aimed to 1. Study the situation of cerebrovascular disease 2. To study the care of stroke patients receiving intermediate care. 3. The subjects were purposively selected as new stroke patients in the first 6 months, 34 patients with ADL scores. less than 15 or greater than or equal to 15. but there are disabilities that are obstacles to the rehabilitation of 2 or more systems, 1 main caregiver in each family, 34 people, community volunteers 34 people, and a multidisciplinary team taking care of 30 patients, totaling 132 people. Symptoms improved by 91.17%. There was one pressure ulcer complication. Representing 2.94% were assessed and diagnosed as disabled at the end of 6 months, 2 cases, representing 5.88%. Patients and families had good quality of life, main caregivers, volunteers and multidisciplinary teams were satisfied. per mode of care at the highest level.
