The Development of Diabetic Care System in community by Transtheoretical Model (TTM), Selaphum Subdistrict Municipality, Selaphum District, Roi Et Province

This research and development aimed to develop a model for diabetic care in the community by using a behavior modification model with the Transtheoretical Model (TTM). This research consisted of 30 people selected by purposive sampling. The research instruments to develop a model for diabetes care in the community are the behavior modification model (Transtheoretical Model: TTM), quantitative data was collected by questionnaires. Data were analyzed by descriptive statistics and Paired samples t-test.
The results of the study showed that after the development of a diabetic care system in the community by using the Transtheoretical Model (TTM), the model of caring for diabetic patients was organized by activities to promote behavior change. according to the differences in the behavioral stages of each patient. The development results showed that the knowledge of diabetes and diabetes control
behavior of diabetic patients after using the behavior modification model was significantly higher than before using the model (t = 6.34, p-value < 0.001 and t = 25.37, p-value < 0.001, respectively). The caregiver's satisfaction with the diabetes care plan in the community was high but the staff’s satisfaction was moderate level. Transtheoretical Model: TTM can be applied to other non-communicable disease (NCD) patients in order to achieve better self-care behavior change.
