This research was research and development with the objective of developing an integrated care model for uncontrolled diabetes patients with community participation. In the diabetes clinic, Nong Phalan Subdistrict Health Promoting Hospital. Sample size were 27 diabetic patients whose disease was not controlled were analyzed using frequency distribution, percentage, standard deviation, Paired t-test statistics, and content analysis.
The research results found that After participating in the activity The sample group had a higher level of knowledge about diabetes and overall quality of life than before participating in the activity at a statistical significance of 0.05 (p < .05) in terms of quality of life, physical and mental health, and social relationships. and environmental increased in every aspect Clinical outcomes have changed for the better. Body Mass Index and cumulative sugar level HbA1C after participating in the activity for 3 months decreased significantly from before participating in the activity at the level of 0.00 (=25.26, S.D.=4.42,
=8.37, S.D.=2.04 ) respectively and were overall satisfied with the development at a high level (
=4.81, S.D.=0.40).
