Nursing care for the patients with acute ischemic stroke who received rt-PA at Kalasin Hospital

This study was case study with the objective of studying nursing care for acute ischemic stroke patients receiving treatment with rt-PA clot-dissolving drug and using the results of the study to develop nursing guidelines for patients with the disease. Acute ischemic stroke treated with rt-PA blood thinners. Comparative study of nursing care for acute ischemic stroke patients receiving treatment with blood thinners. rt-PA who was admitted to the intensive care stroke ward. Kalasin Hospital between May - June 2021, 2 cases. The tools used in the study include Data recording form by collecting information from the patient's medical records. Interview and observation Use data to analyze, compare and plan nursing care using the nursing process.
The results of the study found that Case study 1: Thai male patient, age 66 years. Important symptoms: right limb weakness, lethargy, unable to speak, had this 3 hours before coming to the hospital, history of co-morbidities, diabetes, high blood pressure. The patient has a history of not taking medication continuously. The doctor diagnosed acute cerebral infarction and was treated with blood thinners. The patient had no complications. On the day of discharge, the patient had slight weakness in the right leg and was able to perform daily activities. Case study 2: Thai female patient, 63 years old. Important symptoms include left arm weakness, crooked mouth, stiff tongue, unclear speech. Had this 4 hours before coming to the hospital, history of co-morbidities, diabetes, high blood pressure. The doctor diagnosed acute cerebral infraction and was treated with blood thinners. The patient had complications from receiving blood clot-dissolving drugs, including a minor brain hemorrhage. On the day of discharge, the patient had no remaining disability.
