Nursing Care for Type 2 Diabetic Patients with Stage 3 Chronic Kidney Disease: Two Case Studies

Abstract

     The objective of this case study was to provide the nursing care guidelines for type 2 diabetic patients with stage 3 chronic kidney disease. The two case studies were selected by a purposive sampling method. Data were collected from the medical records of the diabetic patients receiving   treatment at the diabetes clinic. Nursing diagnosis was determined according to Gordon’s 11 functional health patterns to investigate the patients’ problems and set the nursing diagnosis to plan nursing practice, summarize and evaluate nursing outcomes.

     Results: Two type 2 diabetic patients with stage 3 chronic kidney disease were compared.  Both case studies received treatment according to appointments and were screened for diabetes. They had the same comorbidities: high blood pressure and stage 3 chronic kidney disease. However, these two case studies were different in the following aspects. The first patient was female while the second patient was male. They were both elderly people with the factors promoting the disease, namely incorrect self-care behavior about eating and exercise. The second patient had a habit of alcohol drinking and overweight. He had 10 nursing diagnoses and was followed up for 4 times. The second patient had 11 nursing diagnoses and was followed up for 3 times. It was found that both patients still had the habit of eating sweet, fatty, salty food, sticky rice, and seasoning food with fish sauce, fermented fish, and MSG and lacked proper exercise, resulting in being unable to control high blood sugar levels and stage 3 chronic kidney disease, which may lead to faster kidney deterioration.
