NURSEING CARE FOR PULMONARY TUBERCULOSIS WITH PNEUMONIA IN OUTPATIENT SERVICE: CASE STUDY

Abstract

     This case study aimed to study nursing care for pulmonary tuberculosis patient with pneumonia and lacking continuous treatment at the Respiratory Clinic, Nursing Section Outpatient, Kham Thale So Hospital, during 27 June - 26 October 2023. Thai male patient, age 49 years, had pulmonary tuberculosis. The patient took medicine for 1 month but had not taken medicine for 6 months and returned for repeated treatment with symptoms of fever, cough with phlegm, fatigue, and weight loss for 15 days. The doctor diagnosed that the patient had spreading pulmonary tuberculosis with pneumonia, low sodium in the body and was then admitted to the hospital in the isolation room with receiving fluid Therapy and antibiotics and start pulmonary tuberculosis medication, 2IRZE 4IR formula. Nursing problems are including of 1) Risk of spreading tuberculosis during Active TB 2 2) Inefficient breathing due to infection with pulmonary tuberculosis and pneumonia 3) Imbalance of water and mineral salts in the body 4) Impairment in self-care after returning home 5) The patient and relatives are worried about illness. After nursing care, patient is safe, no complications, able to do daily activities and ready to return to take care of themselves after returning home, total of admission in the hospital for 4 days. The patient received antibiotics at the sub-district health promoting hospital for 7 days. The nurse made an appointment to follow up on symptoms. The patient takes medication continuously and can work. Because the patient moved to another province, thus the nurse continues to follow up by Tele-Medicine and send medicine by post.
