The study is an action research (Action Research) with the objective of studying the health conditions of patients with type 2 diabetes at Kuchinarai Crown Prince Hospital Kalasin Province and receiving services according to the guidelines for providing home medicine delivery services for type 2 diabetes patients at Kuchinarai Crown Prince Hospital Kalasin Province The research period is between October 2022 - September 2023, a total of 12 months. Sample size were 983 type 2 diabetic patients receiving services according to the guidelines for providing home medicine delivery services for type 2 diabetic patients at Kuchinarai Crown Prince Hospital Kalasin Province, with inclusion criteria, data collected from medical records. Analyze data by content analysis (Content Analysis), frequency, percentage, mean, standard deviation, and Dependent t – test.
The results of the study found that after the operation, data on Fasting blood sugar (FBS) and HemoglobinA1C (HbA1c) in patients with type 2 diabetes of patients with type 2 diabetes at Kuchinarai Crown Prince Hospital Kalasin Province Receiving the same service (2023) found that Fasting blood sugar (FBS) was normal at 37.03 percent and HemoglobinA1C (HbA1c) was 40.48 percent. In addition, it was found that Diabetic patients with Fasting blood sugar (FBS) less than 180 mg% (n = 886) Fasting blood sugar (FBS) normal 41.08% and HemoglobinA1C (HbA1c) 44.92%. And Fasting blood sugar (FBS) and HemoglobinA1C (HbA1c) in patients with type 2 diabetes at Kuchinarai Crown Prince Hospital Kalasin Province (year 2023) and receiving traditional services (year 2022) are different. Statistically significant at the .05 level, where patients with type 2 diabetes of patients with type 2 diabetes at Kuchinarai Crown Prince Hospital Kalasin Province The average Fasting blood sugar (FBS) and HemoglobinA1C (HbA1c) scores decreased.
