Development of nursing guidelines for patients with end-stage chronic kidney disease to choose continuous kidney replacement therapy in continuity of care to community in Phon Phisai Hospital Nong Khai Province.

This research an action research. Aim to develop and use a guideline for developing nursing for patients with end-stage chronic kidney disease was choose continuous kidney replacement therapy in the community according to the role of continuing care center nurses. Phon Phisai Hospital Nong Khai Province The study was conducted during January-June 2023, sample group of 11 patients with end-stage kidney disease, 11 relatives and caregivers, a multidisciplinary team of 13 people, a total of 37 people. The research tools included an interview form for patients with chronic kidney disease. Patients kidney last stage where therapy is not chosen. Interview form with the multidisciplinary team for continuing care, the need for a model for continuous care at home Interview form with staff members of the network for caring for patients with end-stage chronic kidney disease who do not choose kidney replacement therapy. Data were analyzed using descriptive statistics, percentages, medians, and qualitative content analysis.
The results found that patients with chronic kidney disease receive continuous palliative care services that are appropriate according to the severity of their illness and the need for continued care. Caregivers have knowledge, skills and confidence in caring for patients with chronic kidney disease. Meets the needs of patients with chronic kidney disease, with a high overall level of satisfaction with the continuous care received at 86.96%. They have increased ability to take care of their own health. Initial complications can be managed. When in crisis, you can contact the hospital for timely treatment.
