Development of Intermediate care of stroke patient model in community. using by participation of network partners Mueang Lamphun District, Lamphun Province.

Abstract

     This research was participatory action research aimed to develop and use a model for continuous Intermediate care for stroke patients in the community. using the participation of network partners Mueang Lamphun District, Lamphun Province, studied during the month January-June 2023, a sample group of 145 Intermediate care stroke patients, 145 caregivers, health care team of 11 people. The research tools included an interview form, an interview form for the multidisciplinary team and network partners. Data were analyzed using descriptive statistics, percentages, medians, and qualitative content analysis.
     The results found that After developing the model, it was found that there was increased satisfaction. Before development, the average ADL score before development was 8.86 points, after development it was 9.50 points. When tested with paired t-test statistics, it was found that the average ADL score after developing the model for continuous mid-term stroke care in the community. using the participation of network partners, mean score increased significantly more than before development at the .05 level (t = 2.84).
