Development of a Case Management Model in the care of uncontrolled type2 Diabetic.

Abstract

     This study is an Action Research. Aim to study the effects of Development of a Case Management Model in the care of uncontrolled type2 Diabetic Patients . The sample consisted of 10 diabetic patients with uncontrolled blood sugar levels and complex problems who volunteered to participate in the study. Data collection tools consist of secondary data for gathering medical history, participatory observation, and semi-structured interviews. Outcome measurement tools include a diabetes self-care behavior assessment, a satisfaction survey regarding the case management model, and data analysis using descriptive statistics to determine frequencies, percentages of general data, and qualitative data analysis.
     The results of the development of the case management model show that Patients participating in the program showed a 100% increase in the average score of appropriate behavior. Their HbA1C levels decreased by 100% from before the activity. In the first cycle, the average reduction in HbA1C was 4.77%, and in the second cycle, it decreased by an average of 2.42% compared to the first cycle. Therefore, the case management model is effective in managing patients with blood sugar control issues.
