Evaluation of the drug-resistant tuberculosis surveillance system, Kalasin Province, fiscal year 2022 – 2023.

Abstract

     This report aims to study and review the surveillance system for drug-resistant tuberculosis in Kalasin Province and provide recommendations for the development of the surveillance system for drug-resistant tuberculosis in Kalasin Province. Methodology: This is a mixed methods study divided into 2 parts: 1) This study is a cross-sectional survey research to study the quantitative characteristics of the surveillance system by studying the data of patients who received services at 3 health facilities in Kalasin Province between October 1, 2021 and September 30, 2023. 2) Qualitative study to study the reporting process and qualitative characteristics of the surveillance system for drug-resistant tuberculosis.
     Results: The drug-resistant tuberculosis surveillance system has a sensitivity of 64.0 percent for reporting according to the 2020 disease definition and the 2023 Tuberculosis Investigation and Control Guidelines, and a positive predictive value of 100.0 percent. Data in the disease surveillance system may not be a good representative of all patients found from the medical record review. For data quality, the variables gender, age, and address were 100.0 percent accurate, while variables related to time, such as X-ray date, sputum examination date, registration date, and treatment status, were less than 90.0 percent accurate. As for the timeliness of reporting, it was found that the report was on time within 5 days. For qualitative characteristics, it was found that the work procedures were not complicated and flexible, were accepted and resulted in the stability of the system that could be applied to other surveillance systems. However, due to the workload of the main responsible officers, the data could not be recorded in time. In addition, there was no clear guideline for drug-resistant tuberculosis surveillance for those involved. Conclusion and discussion: The drug-resistant tuberculosis surveillance system of Kalasin Province had a moderate sensitivity of reporting.
