Nursing care for mother post-cesarean section with Severe Pre-Eclampsia, HELLP syndrome, and during Preterm Delivery: A case study

Abstract

     This study was case study aimed to study the nursing care of mothers post-cesarean section with high blood pressure, HELLP syndrome, and preterm birth. A 30-year-old woman, G2 P0 Ab1L0, at 35 weeks of gestation, was admitted to the hospital on March 25, 2023, following a referral from a clinic in Banham district after the detection of fetal intrauterine growth restriction (IUGR) with a rule-out diagnosis of preeclampsia. The physician diagnosed her with severe preeclampsia, with a blood pressure of 188/100 mmHg.
     Results: A cesarean section was performed to deliver the baby. Post-delivery, the mother experienced a 30-second seizure, which posed a risk of tissue hypoxia. She had severe complications of preeclampsia (HELLP syndrome) and was at risk of postpartum hemorrhage in the first 24 hours, as well as wound pain and risk of infection. Nursing interventions included preventing recurrent seizures with 10% MgSO4 (10 g in 5% D/W 1,000 ml IV drip at 1 g/hr.), preventing postpartum hemorrhage, and managing pain. The patient stabilized, was able to start eating, and was encouraged to mobilize. She was able to breastfeed and showed progressive improvement. She was discharged on April 3, 2023, after a total hospital stay of 9 days.
