Delvelopment of a care model for pulmonary tuberculosis patients in Phayakkhaphumphisai District Mahasarakham Province.

This participatory action research aimed to develop a model for caring for pulmonary tuberculosis patients at Phayakkhaphumiphisai Hospital, Maha Sarakham Province, consisting of 3 phases: 1) Phase 1, studying the situation of caring for pulmonary tuberculosis patients in the area, 2) Phase 2, developing guidelines for caring for pulmonary tuberculosis patients in the area, and 3) Phase 3, evaluating the results of using the guidelines for caring for pulmonary tuberculosis patients. The study was conducted between 1 October 2021 and 30 September 2022. Data were collected using in-depth interviews, focus group discussions, and questionnaires. Data were analyzed using content analysis, descriptive statistics, and Paired t-test.
The study results found that the development of guidelines for the care of pulmonary tuberculosis patients in Phayakkhaphumiphisai District consisted of (1) service and care management, (2) developing the potential of personnel and teams involved in care, (3) developing a database system, communication, and surveillance between the service team and the community, (4) organizing a drug service system and technology for monitoring drug use, (5) supporting a budget for patient care, assistance funds, and social support sources, (6) setting policies and management plans at the district level, creating network participation, and (7) community participation. This development resulted in an innovative 2x2x2 model strategy. After developing the care guidelines, patients' self-care behavior changed significantly for the better (p<0.001) in all aspects. Patients' overall satisfaction increased to a very high level. The mean score was 3.36 (S.D.=0.20), and the treatment success rate (Success Rate) increased. The drug dropout rate and the death rate of tuberculosis patients decreased.
