Comprehensive dental care is a treatment approach that considers patients' needs alongside their medical limitations, psychological state, and social, and economic conditions, with an emphasis on sustainable oral health maintenance. This case report presents the treatment of a 73-year-old Thai female patient with hypertension and hyperlipidemia, who sought dental treatment due to a lack of occlusal support. Oral examination revealed mandibular tori on the lingual aspect and large buccal exostoses, which posed challenges for denture fabrication. The treatment plan began with oral hygiene control and periodontal therapy, followed by pre-prosthetic surgery to prepare the oral cavity for denture placement. The surgery involved the removal of mandibular tori and buccal exostoses. After surgical wound healing, a single maxillary complete denture opposing a mandibular extended-base removable partial denture was fabricated, incorporating bilateral balanced occlusion to prevent combination syndrome.
Six-month follow-up showed good surgical wound healing with no inflammation or bone regrowth. The dentures demonstrated good retention and stability without complications. The patient achieved efficient masticatory function, resulting in improved quality of life. Furthermore, the patient developed a positive attitude toward oral health care and maintained regular follow-up visits. This report highlights the importance of holistic treatment planning and continuous follow-up care in managing complex dental cases.
