The nursing care of patient with Diabetes Mellitus and End Stage of Renal Disease at Nonsila Hospital: 2 cases studies

Abstract

     This study is a case study aimed at examining the nursing care of patients by selecting two patients with end-stage chronic kidney disease who received services at the Chronic Non-Communicable Disease Clinic of Non Sila Hospital. The purpose is to compare the symptoms, treatments, complications of the disease, and nursing approaches in diabetic patients undergoing hemodialysis with those receiving continuous ambulatory peritoneal dialysis. Data was collected from medical records, interviews with patients and relatives, along with observations. Nursing diagnoses were established, and nursing care plans were developed, implemented, and evaluated for outcomes. The study utilized Gordon's functional health patterns as a framework for analysis and conducted a content analysis of the data.
     Results: Both patients are suffering from complications related to diabetes and end-stage chronic kidney disease. Patient 1 has complications from end-stage chronic kidney disease and is being treated with dialysis using a dialysis machine. Their blood sugar levels are high due to inappropriate dietary control, but they are becoming more self-sufficient. They still experience anemia, and the doctor has prescribed medication to increase blood concentration. Fatigue and dizziness have decreased. Patient 2 has diabetes and is being treated with dietary control, as well as continuous ambulatory peritoneal dialysis. Abdominal pain has decreased, there are no signs of infection in the abdominal cavity, and fatigue has lessened, with normal breathing.
