Development of Nursing Practice Guidelines in Caring for Patients Undergoing Open Heart Surgery for Operating Room Nurses.

This research and development study aimed to develop and evaluate the implementation of a nursing practice guideline for open-heart surgery patients by operating room nurses. The study was conducted in four phases: (1) situational analysis, (2) guideline development, (3) implementation of the guideline in patient care, and (4) evaluation. A purposive sample included 38 registered nurses and 80 patients undergoing open-heart surgery. Research instruments included the nursing practice guideline (IOC = 0.93) and an implementation manual (AGREE II score = 89.67%). Data collection tools consisted of a knowledge test (KR-20 = 0.81), a competency assessment form, a nurse opinion survey, a patient satisfaction questionnaire (Cronbach’s alpha = 0.89, 0.84, and 0.88, respectively), and a clinical outcome recording form (IOC = 0.93). Data were analyzed using descriptive statistics, paired t-test, independent t-test, and Chi-square test at a significance level of .05.
Results indicated that the nursing practice guideline consisted of four components: (1) preoperative care, (2) intraoperative care, (3) postoperative care, and (4) discharge planning. Post-implementation, nurses demonstrated significantly improved knowledge and competency, along with highly favorable opinions toward the guideline. Patients reported high satisfaction with the care received. Furthermore, there were statistically significant reductions in postoperative blood loss, surgery postponements due to poor patient preparation, surgical wound infections, treatment costs, and average length of stay in the ICU (p < .05).
