Prevalence and quality of life of elderly with osteoarthritis at the chronic disease clinic, Manang Hospital, Satun Province

Abstract

     This study aimed to (1) investigate the prevalence of knee osteoarthritis among elderly individuals attending the chronic disease clinic at Manang Hospital, Satun Province, (2) evaluate the quality of life of elderly patients with knee osteoarthritis according to disease severity, and (3) examine demographic, health, and behavioral factors associated with knee osteoarthritis and quality of life. A survey research by cross-sectional design was employed. The sample consisted of 308 elderly individuals aged 60 years and above with chronic diseases, selected through simple random sampling. Research instruments included a general information questionnaire, the Thai version of the Oxford Knee Score, and the Thai version of the EQ-5D-5L quality of life questionnaire. Data were analyzed using descriptive statistics, chi-square test, t-test, logistic regression, and multiple linear regression.

     The results revealed that 35.1% of elderly participants experienced chronic knee pain (95% CI: 29.9–40.5), yet only 8.8% had received an official diagnosis. Among those reporting knee pain, only 25.0% had been diagnosed, indicating a diagnostic gap of 75.0%. Participants with knee pain had significantly lower quality of life scores (VAS Score) than those without pain (71.07±14.79 vs. 81.97±10.22, p<0.001). The primary risk factor identified was having an above-standard body mass index (Adjusted OR = 3.52, 95% CI: 2.11–5.87). Positive associations with quality of life were found in participants who exercised regularly and had higher Oxford Knee Scores. A significant positive correlation between Oxford Knee Score and VAS Score was observed (r = 0.492, p<0.001).
