Development of a Model for Continuing Palliative Care from hospital to community Pra Arjanfan Arjaro Hospital

Abstract

     This research was Research and Development of aimed to Development of a Model for Continuing Palliative Care from hospital to community Pra Arjanfan Arjaro Hospital. The Sample were 42 End-of-life patient and Multidisciplinary team. Research Instrument by 1) Model for Continuing Palliative Care from hospital to community  2) The Patient Care Data Collection Form 3) The Questionnair of Patient Satisfaction 4 The Questionnair of Multidisciplinary Team Satisfaction. Data were analyzed by frequenzy, mean , standard deviation , content analysis and Independent t-test.

     The result were show  1) Following the implementation of the palliative care model, there was a statistically significant increase (p<.05) in the assessment of patients using PPS and ESAS, as well as in the management of disturbing symptoms 2) The results showed that after implementing the Advance Care Planning (ACP) model during community home visits, palliative care patients significantly chose to die at home, (p<.05). 3) Patients and their families reported a high level of satisfaction with the mode 4) The multidisciplinary team reported a high level of satisfaction with the model.
