Abstract

     This study aimed to identify factors associated with death during treatment among pulmonary tuberculosis (TB) patients in Public Health Region 6 of Thailand. A survey research by retrospective cohort study was conducted using secondary data from the National Tuberculosis Information Program (NTIP). The study population included patients registered for pulmonary TB treatment across all healthcare facilities within Health Region 6 between October 1, 2023, and September 30, 2024. Patients who were still undergoing treatment, had a revised diagnosis, were re-registered as drug-resistant TB, or died from external causes such as accidents or suicide were excluded. Of 6,258 eligible patients, 5,684 (90.8%) survived and 574 (9.2%) died during treatment. Adjusted relative risks (aRR) and 95% confidence intervals (CI) were estimated using modified Poisson regression with robust error variance. Multivariable analysis identified the following independent risk factors for mortality: age > 60 years (aRR 5.53; 95% CI 3.74–8.16), male (aRR 1.22; 95% CI 1.02–1.45), body mass index (BMI) <18.5 kg/m² (aRR 1.39; 95% CI 1.21–1.70), alcohol consumption (aRR 2.09; 95% CI 1.32–3.32), HIV co-infection (aRR 2.90; 95% CI 2.26–3.72), chronic kidney disease (aRR 2.32; 95% CI 1.69–3.18), diabetes mellitus (aRR 1.43; 95% CI 1.18–1.75), chronic obstructive pulmonary disease (COPD) (aRR 1.42; 95% CI 1.03–1.96), and sputum smear positivity before treatment (aRR 1.44; 95% CI 1.21–1.71).
