Prevalence and Characteristics of Masked Hypertension and White Coat Hypertension Using Community-Based Blood Pressure Monitoring Compared to Hospital-Based Measurements: A Cross-Sectional Study in High-Risk Population.

Abstract

      This study aimed to determine the prevalence of MH and WCH among individuals at risk of hypertension by comparing BP measurements obtained at the Thai People Away from NCDs Centers and those taken in a healthcare facility, and to compare the mean BP values between the two settings. A survey research by cross-sectional study was conducted among 218 individuals aged 35-59 years at risk for hypertension, residing in the service area of Ban Khok Subdistrict Health Promoting Hospital, Nong Han District, Udon Thani Province, from August to October 2025. BP measurements were performed at the community centers for 7 consecutive days, twice daily, and at the hospital on 3 occasions, following the 2023 European Society of Hypertension guidelines.5 Data were analyzed using descriptive statistics and paired t-tests.

      Results : The majority of participants were female (61.0%), with a mean age of 47.3±7.2 years and a mean BMI of 26.3±4.1 kg/m². Of these, 65.1% were overweight. The most common risk factors included high intake of salty and sweet foods (81.7%), physical inactivity (71.6%), and overweight (65.1%). BP classification revealed sustained normotension (40.8%), WCH (19.3%; 95% CI: 14.0-24.5), MH (14.2%; 95% CI: 9.6-19.9), and sustained hypertension (25.7%). Mean office BP values were significantly higher than those measured at the community centers: systolic BP (142.8±15.6 vs. 135.4±12.3 mmHg, p<0.001) and diastolic BP (89.2±8.4 vs. 84.7±7.1 mmHg, p<0.001), with mean differences of 7.4 mmHg and 4.5 mmHg, respectively.
