The development of a seamless community-participatory mental health care model in Mueang Suang District, Roi Et Province aimed to: 1) study the situation and problems of psychiatric patient care; 2) develop a seamless community-participatory mental health care model; and 3) evaluate outcomes of the developed model. This was a participatory action research (PAR) conducted in three phases. The sample group consisted of 10 psychiatric patients and 10 relatives or caregivers of psychiatric patients. Data were analyzed using descriptive statistics and inferential statistics with Chi-square test.
The findings revealed that psychiatric care problems existed across all three phases: pre-hospital, in-hospital, and post-hospital. The developed model comprised five components: 1) integrated care network; 2) personnel training; 3) mental health prevention and promotion activities; 4) emergency care and referral system; and 5) digital technology for patient follow-up. After model implementation, the community mental health screening rate significantly increased from 10.90% to 18.41% (p<0.001), screening coverage for seven risk groups increased from 60.00% to 80.02% (p<0.001), continuity of care for schizophrenia patients increased from 58.24% to 80.22% (p<0.001), the follow-up rate for missed appointments increased from 70.40% to 90.20% (p<0.001), and proactive home visits increased by 86.67% (from 150 to 280 visits). Overall stakeholder satisfaction with the model was at a high level (Mean=4.38).
