Nursing care for pregnant women with hypertension : Two case studies.

Abstract

      A Comparative Case Study of Nursing Care for Full-Term Maternal Patients with Severe Preeclampsia: Pre-delivery to Postpartum Period. This comparative case study aimed to analyze and compare the nursing care provided to two primigravida (first-time mothers) diagnosed with Severe Preeclampsia and postpartum hypertension. Both patients were treated at Kalasin Hospital within the Labor Room, obstetric-gynecological wards, and the special obstetric unit. The scope of care encompassed the prenatal, intrapartum, and postpartum periods, including comprehensive discharge planning. Data were collected through clinical assessments, patient and relative interviews, and medical record reviews. Nursing diagnoses and care plans were established based on Gordon’s 11 Functional Health Patterns. Clinical outcomes were evaluated to determine the effectiveness of the multidisciplinary care provided.

      Results:

Case Study 1: A primigravida diagnosed with Severe Preeclampsia complicated by fetal hypoxia, necessitating an emergency cesarean section. Intensive management included blood pressure control, seizure prophylaxis with Magnesium Sulfate, and blood transfusions to correct anemia. Co-existing Hyperthyroidism was identified and managed via a multidisciplinary approach. The patient responded well to treatment, and both mother and infant were safely discharged.

Case Study 2: A primigravida at 34 weeks of gestation with Severe Preeclampsia. Management focused on standardized seizure prophylaxis and intravenous antihypertensive therapy. Pregnancy was terminated via cesarean section upon clinical indication. Both mother and infant remained safe without severe complications and were discharged in good condition.
