The Development of an advance care plan model for palliative care patients, Nong Song Hong Hospital, Khon Kaen Province.

Abstract

      This study aimed to : 1) study and analyze the situation of advance care plan for palliative care patients, Nong Song Hong Hospital, Khon Kaen Province. 2) develop a care model for advance care plan for palliative care patients, Nong Song Hong District, Khon Kaen Province. 3. To study the results of designing an advance care plan for palliative care patients in Nong Song Hong District, Khon Kaen Province. This study was an action research. 1. Selected a sample group of 40 patients diagnosed as palliative patients. 2. 40 relatives or caregivers of the patients who had the authority to make decisions on behalf of the patients. It was divided into 3 steps: preparation phase, the review of the situation regarding the previous model of good death. Implementation phase, model development, evaluation, patient-family discussions, care planning in the hospital and after discharge, and home visit follow-up. Evaluation phase, the changes in pain level was analyzed using Paired t-test and the satisfaction level of patient care services was analyzed using descriptive statistics, percentage, and standard deviation.

      Results: It was found that the mean pain scores before and after were significantly different (P< 0.001) and the satisfaction level of relatives in receiving palliative services was very satisfactory.
