Nursing care for patients with acute heart failure due to a hypertensive crisis with acute respiratory failure : case study.

This study was case study. The objective was to study nursing care for patients with acute heart failure due to a hypertensive crisis with acute respiratory failure. The study was conducted on a 39-year -old female patient with past illnesses of diabetes, hypertension, and dyslipidemia. She presented with symptoms of breathlessness and palpitations six hours before the emergency room at the community hospital. During the initial assessment and treatment at a community hospital, she exhibited dyspnea, hypertension crisis, hyperglycemia, and tachycardia. The doctor’s diagnosis included acute heart failure, acute respiratory failure, and Supraventricular tachycardia. Initial interventions involved endotracheal intubation with manual Ambu bag, intravenous diuretics, antihypertensive drugs, anti-arrhythmic drugs, synchronized cardioversion, and referral to the provincial hospital
The study results, during her stay at the provincial hospital, several clinical challenges were identified, including impaired gas exchange, persistent hypertension, hyperglycemia, tachycardia, respiratory tract infection, acute kidney injury (AKI), hypokalemia, urinary tract infection (UTI), and dehydration. Key nursing focused on mechanical ventilator management, continuous assessment of vital signs and oxygen saturation, monitoring blood glucose levels and maintaining fluid and electrolyte balance, and administration of intravenous antihypertensives, diuretics, and antibiotics as prescribed. Following a comprehensive treatment and nursing management, the patient’s condition improved. She was successfully discharged after a total length of stay of 11 days.
