The Effects of Nursing Practice Guidelines for Preventing Complications among Pregnant Women with Preeclampsia in the Labor Room at Phayao Hospital

Abstract

     This quasi-experimental study with a two-group comparative design using a historical control evaluated the effectiveness of a developed nursing practice guideline in reducing complications among pregnant women with preeclampsia. The study included 116 pregnant women diagnosed with preeclampsia between February 2024 and July 2025, divided into a control group receiving standard care (n = 58) and an intervention group receiving the developed guideline (n = 58), along with 8 nurses. The nursing practice guideline was developed based on Soukup’s Evidence-Based Nursing Practice Model as the conceptual framework and its content validity was confirmed by three experts (CVI = 0.80). Data collection instruments included: (1) a structured record form comprising demographic data, obstetric history, physical examination findings, and clinical outcomes; and (2) a nurse satisfaction questionnaire assessing the acceptability of the guideline. Data were analyzed using descriptive statistics, Fisher’s exact test, and multivariable logistic regression.

     The results showed that the complication rate differed significantly between the two groups (p < 0.05). After adjusting for underlying medical conditions and income, the use of the guideline was associated with a significant reduction in complications (adjusted OR = 0.37; 95% CI: 0.15–0.92; p = 0.033). Nurses reported a high level of satisfaction with the guideline (mean = 4.28, SD = 0.69).
