A model for developing integrated elderly clinic services in Ubolratana District, Khon Kaen Province.

This research is a Research and Development (R&D) study utilizing a Participatory Action Research (PAR) approach. It aims to develop an integrated geriatric care clinic service model and evaluate its outcomes in terms of screening rates, service accessibility, and satisfaction. The implementation was divided into 3 phases: 1) studying the local situation and context; 2) developing the model collaboratively with network partners by utilizing a proactive mobile geriatric clinic and a Telemedicine system; and 3) evaluating the operational outcomes. The sample consisted of 153 personnel and network members, along with the elderly population in Ubolratana District. Quantitative data were analyzed using descriptive statistics integrated with qualitative content analysis.
The results revealed that the developed service model consists of 5 key components: proactive screening, reassessment, mobile clinic and treatment, Telemedicine, and referral for continuous care. When comparing the outcomes before and after the model implementation, it was found that the health risk screening rate increased from 56.61% to 82.00%. The service access rate for at-risk elderly individuals increased from 15.40% to 93.10%. The specialist referral rate increased from 4.80% to 21.00%. Furthermore, the satisfaction level toward the service model increased from a moderate level (3.42) to the highest level (4.68) with statistical significance (p < 0.01). However, significant barriers to treatment referrals were identified; namely, the majority of the elderly lacked caregivers (No Caregiver) and faced technological limitations in using the Telemedicine system independently
