Strengthening Community-Based Tuberculosis Management : From Active Case Finding to Seamless Referral Systems by Health Networks.

Abstract

      This action aesearch aimed to investigate situational needs, develop a management model, and evaluate the effectiveness of an enhanced community-based tuberculosis (TB) program. Sample size were 442 target participants. Data were collected using knowledge tests, questionnaires, and research activity logs between March 2025 and March 2026. Statistical analyses were performed using descriptive statistics, Z-test, and content analysis.

      The results revealed:

      1) Situational Analysis: Significant barriers were identified within the passive case-finding system and the shortage of radiologists. These factors contributed to diagnostic delays and increased transmission risks among hard-to-reach populations.

      2) Model Components: The developed management model consists of five core components: (1) Proactive Planning, (2) Community Engagement, (3) Mobile Chest X-ray (CXR) Screening, (4) Laboratory Diagnosis, and (5) Seamless Continuum of Care and Treatment.

      3) Quantitative Outcomes: Post-implementation, the target group demonstrated significantly improved TB knowledge and preventive behaviors. The screening access rate during the 2025–2026 fiscal years increased significantly (Z = 260.50, p < .001), with 50,676 individuals (97.08% of the high-risk group) successfully undergoing chest X-rays. The model identified and enrolled 170 new TB cases into the treatment system, representing a diagnostic yield of 0.33%. Furthermore, the overall standards and efficiency of the model were rated at a high level.
