Factors affecting medication use behavior among elderly patients With hypertension in Sa Kaeo Province.

The objectives of this descriptive study aimed to investigate personal profiles, medication knowledge, health beliefs, healthcare systems, social support, medication adherence behaviors, and blood pressure levels among elderly patients with hypertension in Sa Kaeo Province. It further compared adherence behaviors and blood pressure levels based on relevant factors, and analyzed the influencing factors to predict medication adherence. The sample consisted of 380 participants. The research instrument was an interview questionnaire with an Index of Item-Objective Congruence of 1.00 and an overall reliability coefficient of 0.83. Data were analyzed using descriptive statistics and inferential statistics.
The results revealed that the majority of participants were female, aged 60 to 69 years, completed primary education, and engaged primarily in agriculture. The overall medication adherence behavior score was at a high level with a mean of 34.62, and 79.74 percent successfully achieved controlled blood pressure levels. Hypotheses testing demonstrated that educational attainment and primary occupation significantly differentiated medication adherence behaviors, whereas gender and age yielded no significant differences. Furthermore, distinct tiers of knowledge, health beliefs, social support, and healthcare system perceptions significantly altered adherence profiles, showing that the adherent group achieved better blood pressure control than the non-adherent group. Ultimately, healthcare system perceptions and social support emerged as the primary significant positive predictors that co-predicted medication adherence behaviors at a significance level of 0.05. Therefore, the healthcare approach should focus on proactive service delivery integrated with family and Village Health Volunteer networks. This includes utilizing public health information systems to manage individualized patient registries with real-time data connectivity, as well as developing digital medication reminders and appointment notification systems to empower healthcare teams to monitor medication adherence with greater precision.
