Quality Improvement of Nursing Documentation, Male Medical Ward 1, Lampang Hospital

This study aimed to improve the quality of nursing documentation in Male Medical Ward 1, Lampang Hospital, by applying the Continuous Quality Improvement (CQI) process through the FOCUS-PDCA framework. This action research utilized a pre-post design based on the 9-step FOCUS-PDCA process. The population consisted of 19 professional nurses, and data were collected from a sample of 58 medical records (29 pre-intervention and 29 post-intervention). The research instruments, validated by three experts with a Content Validity Index (CVI) of 1.0, included: 1) Focus group discussion guidelines concerning documentation issues, 2) A nursing documentation manual developed by the researcher following the APIE nursing process and Focus Charting format, and 3) A modified APIE nursing documentation audit form. Data were analyzed using descriptive statistics, including frequency, mean, and percentage.
Results: Following the implementation of the FOCUS-PDCA process, the accuracy and completeness of the Focus List and Progress Notes (recorded in APIE format) increased from 72.4% to 93.1%. Additionally, there was a positive trend in spiritual dimension recording, which increased from 0% to 58.6%.
