A Structural Equation Model of Digital Health Literacy and Factors Influencing Chronic Disease Self-Management Support Behaviors of Village Health Volunteers for Older Adults

Abstract

     This study aimed to develop and examine a structural equation model of digital health literacy, participation, social support, self-efficacy, and role perception influencing chronic disease self-management support behaviors among village health volunteers (VHVs) caring for older adults. A survey research by cross-sectional design was employed. The participants were 636 VHVs in Khlong Luang District, Pathum Thani Province, Thailand. Data were collected using a structured questionnaire covering six major constructs and analyzed using descriptive statistics, Pearson’s correlation coefficients, confirmatory factor analysis, and structural equation modeling.

     The results showed that social support had the highest mean score (Mean = 4.27, SD = 0.45), followed by role perception (Mean = 4.24, SD = 0.49). All study variables were positively correlated with chronic disease self-management support behaviors (p < .001), with participation demonstrating the strongest correlation (r = .563). Structural equation modeling showed that social support (β = .531, p < .001) and digital health literacy (β = .199, p < .001) had significant positive direct effects on participation. Participation (β = .265, p = .004) and role perception (β = .211, p = .036) had significant positive direct effects on chronic disease self-management support behaviors. Digital health literacy and social support also contributed to support behaviors through participation, indicating the important mediating role of participation in translating individual and social resources into practice. The model explained 79.8% of the variance in participation and 40.0% of the variance in chronic disease self-management support behaviors.
