Abstract

This study aimed to study the existing situation, develop and evaluate a care system, and follow up on the implementation of the care system for patients with hypertension undergoing Thai traditional medicine procedures at Don Mod Daeng Hospital. This research was quasi experimental research. Sample size were 68 purposively selected patients with hypertension aged 40 years or older. The research data were collected from May 1, 2025, to July 31, 2025, using a one-group pretest-posttest design. The implementation of the system as a routine service guideline was subsequently followed up from August 1, 2025, to July 31, 2026, for a total of one year. The research instruments were examined for content validity. Data were analyzed using descriptive statistics, content analysis, and a paired-samples t-test.

Results: The developed care system consisted of health and risk assessment, blood pressure measurement and interpretation, preparation and reassessment, criteria for providing procedures, withholding procedures, and referral, monitoring during and after the procedures, documentation, and patient education. The content validity index was 0.92, and the satisfaction and confidence questionnaire demonstrated a Cronbach’s alpha coefficient of 0.89. Among the 68 patients, complete health assessment and documentation according to the system were achieved in 100% of cases, and no adverse events were observed. The mean systolic blood pressure decreased significantly from 145.30 to 137.50 mmHg (t(67) = 3.21, P-value = .002), while the mean diastolic blood pressure decreased significantly from 91.60 to 87.00 mmHg (t(67) = 2.47, P-value = .016). Overall patient satisfaction and confidence were at the highest level, with a mean score of 4.55 and a standard deviation of 0.55. Following the evaluation, the system was implemented as a routine service guideline continuously from August 1, 2025, to July 31, 2026, for one year.
