Association Between Clinical Presentations and Substance Use Patterns Among Emergency Department Patients: A Single-Center Study

Abstract

      This study aimed to examine personal factors associated with substance use among patients admitted to the Emergency Department, King Chulalongkorn Memorial Hospital. And, to investigate clinical presentations associated with substance use among patients admitted to the Emergency Department, King Chulalongkorn Memorial Hospital. This survey research by retrospective study was conducted at the Emergency Department of King Chulalongkorn Memorial Hospital, Thai Red Cross Society. Medical records from a four-year period (January 2022 to December 2025) were reviewed. The target population consisted of emergency patients diagnosed with mental and behavioral disorders due to psychoactive substance use (ICD-10-TM codes: F10–F19). A total of 126 patients met the eligibility criteria. Data were analyzed using mean, standard deviation, and the Chi-square test.

      Results: Regarding clinical presentation, the most common chief complaints were agitation/psychiatric symptoms (32.54%) and palpitations/chest tightness (27.78%). Most patients had no underlying medical comorbidities (42.86%), while 32.54% had co-occurring psychiatric disorders. Emergency triage classification revealed that the majority were categorized as Urgent (46.83%), followed by Emergency (35.71%). Regarding substance involvement, a history of amphetamine use was most often reported (45.24%). Laboratory toxicological screening detected substances in 38.89% of cases (predominantly amphetamines at 22.22%), whereas 61.11% tested negative. Concerning treatment outcomes, 57.94% of patients were admitted to the hospital, and 32.54% were discharged home. Occupation and a reported history of substance use were significantly associated with positive laboratory drug screening results (p < 0.05). Furthermore, underlying medical conditions, chief complaints, triage acuity levels, and history of substance use were significantly associated with clinical treatment outcomes (p < 0.05).
