Development of a Nursing Care Model for Preventing Venous Thromboembolism among Patients Undergoing Total Knee and Hip Arthroplasty at Phayakkhaphum Phisai Hospital.

This research and development study aimed to examine the situation, develop a nursing care model, and evaluate its effectiveness in preventing venous thromboembolism (VTE) among patients undergoing total knee and hip arthroplasty. The design was applied together with Soukup's evidence-based practice model, the PDCA cycle, and the Caprini risk assessment model. Participants included 20 multidisciplinary team members, 15 patients in the pilot phase, and 40 patients in the evaluation phase. Outcomes were compared with a historical cohort of 65 patients. Data were analyzed using descriptive statistics, the chi-square test, Fisher's exact test, and content analysis.
The resulting PYK-VTE Model comprised five components: scheduled Caprini risk assessment, risk-stratified prophylaxis bundles, early mobilization with ankle exercises, patient and family education, and shift-based surveillance with discharge planning. After implementation, guideline adherence increased from 41.5% to 92.5% (p < .001), and mean time to first ambulation decreased from 36.2 to 21.4 hours. No VTE occurred in the evaluation group, compared with 3.08% in the historical group, although the difference was not statistically significant (p = .52). Nurse satisfaction was high (M = 4.46, SD = 0.48).
