Abstract

      This research and development study aimed to examine the situation, develop, and evaluate a hospital-to-home continuity of care model for newborns with jaundice (JAUNDICE-CARE Plus). Soukup's evidence-based practice model from October 2025 to July 2026. Participants were 20 health care providers, newborns with a gestational age of 35 weeks or more with jaundice (40 before and 43 after implementation), and their primary caregivers. Instruments were validated for content validity (CVI = 0.92). Data were analyzed using descriptive statistics, the chi-square test, Fisher's exact test, the independent t-test, the paired t-test, and content analysis.

      The model consisted of three phases and nine components: the in-hospital phase, the discharge preparation phase, and the home follow-up phase. After implementation, the rate of delayed diagnosis or treatment decreased from 20.0% to 4.7% (p = 0.044), the rate of on-time follow-up increased from 57.5% to 90.7% (p < 0.001), and the mean length of stay decreased from 3.7 to 2.9 days (p = 0.002). The 7-day readmission rate decreased from 10.0% to 2.3%, although this difference was not statistically significant, and no cases of kernicterus occurred. Caregivers' knowledge and competency scores increased significantly after coaching (p < 0.001). Satisfaction was at the highest level among caregivers and at a high level among providers.
