This research was a participatory action research. The objective was to study the health status and context of stroke patients. The process of continuing care for the patient at home The role of the multidisciplinary team The role of network partners in the community and the effect of participatory home care process for subacute stroke patients at Khao Wong Hospital The sample consisted of A multidisciplinary team of 13 people and 57 members of Song Nuea sub-district network associates, a total of 70 people. Data were collected from care results records. and an understanding of the roles of multidisciplinary teams and network partners between October 2021 – September 2022, a total of 12 months. Data analysis by Descriptive statistics were frequency distribution, percentage, arithmetic mean. and standard deviation Inferential statistics were used, F-test, One way ANOVA, Dependent t-test and Content Analysis.
The results showed that Home care process for subacute stroke patients by a multidisciplinary team and network partners in the community to participate in patient registration, coordinating multidisciplinary teams and network partners in the community, preparing houses, communities and relatives or carers, planning continuation of care for patients at home, taking care of patients sick according to home care plans and follow up and assess the rehabilitation results of patients at home resulting in continuous care of subacute stroke patients at home during the fiscal year 2022 (Oct. 21-Sep. Suffering from subacute stroke The after operation was higher than before the statistical significance at the .05 level, including the multidisciplinary team. And network partners have a better understanding of the role of continuous care of subacute stroke patients at home.
