Abstract

          The study entitled “The study entitled “Participatory Action Research for Developing Mechanism of Case Management for Elderly Care in Bang Si Thong Sub-district, Nonthaburi Province” the result that the process of case management for elderly started from finding the elderly with complicated needs, assessing of individual needs, planning, Implementing, reviewing, case closure. The model of elderly case management in research area was found into 2 levels: 1) Administrative case management level for the elderly who could care himself/herself and 2) Resource coordination level for the elderly with severe problems that must be cured or rehabilitated. However, the issue of mechanism of existing case management for elderly care in Bang Si Thong Sub-district wasn’t clear and this leaded to develop new mechanism to support case management such as development service packages for the elderly. Preparation of service plan finding the elderly who needy and patients with chronic disease and setting volunteer system for case management process that would lead to develop individual elderly quality of life effectively.
