IMPROVEMENT OF A CONTINUING CARE MODEL OF THE HOME-BOUND AND BED-BOUND STROKE’S PATIENTS IN THUNG FON HOSPITAL, UDON THANI PROVINCE

Abstract
This study aims to participatory action research process. The duration of the study from April 2023 to August 2023 was carried out in 3 phases, namely 1) the assessment phase 2) The development phase of the model, which is considered as the operation of 3 operations cycles, and 3) the dpatients 2) 35 Village Health Volunteers (VHV) 3) 7 Registered Nurses at Health Promoting Hospital 4) 3 persons responsible for chronic disease at Thung Fon Hospital and 5) A multidisciplinary team consisting of physician, pharmacist, nutritionist, public health academic, and Thai traditional medicine total of 5 people. The researcher has collected data and facilitated the discussion and brainstorming sessions. The statistics were used in the analysis of quantitative data by frequency and percentage, while the qualitative data used content analysis and the participants were involved in the analysis in every process. The results of the research showed that;
All 13 home-bound and Bed-bound Stroke Patients were 8 males (61.54%) and 5 females (38.46%) and they were evaluated by Activities of Daily Living (ADL) for assessment functional independence namely, 9 persons (69.24%) total dependence, 4 persons (30.76%) severe dependence. Besides, participants were appraised for losing the ability in activities of daily living which found out the 11 samples (84.62%) are most impairment the ability of daily living and the 2 subjects (15.38%) are more decline in the ability of daily living
Continuing Care Model of the Home-bound and Bed-bound Stroke’s Patients in Thung Fon Hospital try design out, applied concepts, exchanging experiences, reflections and developed the model as follows: 1) Obvious model and mutual awareness 2) Rules for working with multidisciplinary teams and networks 3) Able to solve problems and provide comprehensive care for stroke patients.
