EFFECTS OF CASE MANAGEMENT PROGRAM IN ELDERLY CHRONIC EYE UNCONTROLLED BLOOD SUGAR AND BLOOD PRESSURE

Abstract
This experimental study is a qualitative research study with an in-depth analysis of three cases. Every case study used a case management model consisting of patient care planning steps. Coordination of monitoring, supervision, and evaluation of sample cases to be studied is a person over the age of 65 who has been diagnosed with diabetic retinopathy. who are at high risk of losing control of their blood sugar and blood pressure levels? Is a patient receiving services in the Retina and Vitreous Center, Floor M, Lertpracharak Building, after following the case study for three rounds of appointments and continuing to give nursing care at home? Mettapracharak (Wat Rai Khing) Hospital. The results of the study found that the sample group received nursing care using the case nursing management model. That consists of nursing activities and knowledge to change behavior. Creating motivation to change behavior together with the healthcare team Evaluation and planning of individualized care together; counseling continuous, excellent follow-up Can control blood sugar levels and blood pressure levels. Can reduce the severity of complications. There were no retinal detachments, and we were satisfied with the case management.
